SPRINKLE HEALTH MEMBERSHIP AGREEMENT — Version 2026-10-v6

1. Membership. Sprinkle Health provides a household membership giving access to telehealth and, for Personal Primary Care, direct primary care services. Charges are per household, not per person; adding dependents does not increase the membership charge. For new enrollments under this agreement, Everyday Telehealth is $19.99 plus $1.89 Priority Membership per month ($21.88 total; $262.56 annually). Personal Primary Care is $29.99 plus $2.34 Priority Access per month ($32.33 total; $387.96 annually). Monthly enrollment adds a one-time $49 onboarding fee: Personal Primary Care is $81.33 at checkout, followed by 11 monthly installments of $32.33, totaling $436.96 for the initial term. Annual prepayment includes Priority Access for all 12 months and waives onboarding. Previously enrolled memberships retain their accepted prices and terms. Sprinkle Health is not insurance. All medical services are provided by independent licensed providers.
2. Initial term. Each membership has an initial 12-month term, paid either in 12 monthly installments (the first collected at enrollment together with the one-time onboarding fee) or in one annual payment with the onboarding fee waived.
3. Automatic renewal. At the end of the initial 12-month term, whether paid in 12 monthly installments or one annual payment, the membership automatically switches to month-to-month billing at the then-current applicable monthly rates, including applicable recurring fees and add-ons, until canceled. There is no second 12-month commitment. We will remind you at least 30 days before an annual membership switches to monthly billing and at least 7 days before a monthly renewal charge, including the amount, date and how to cancel. The onboarding fee is never charged again.
4. Cancellation. Monthly memberships cannot be canceled during the initial 12-month commitment; all 12 installments are due. After the initial 12 months, you may cancel your next automatic monthly payment through your member portal or by calling Sprinkle Health. Annual members may cancel at any time before the switch to monthly billing; cancellation takes effect at the end of the paid year. This does not limit any rights you have under applicable state or federal law.
5. Refunds. All charges are nonrefundable except where required under applicable state or federal law. Canceling automatic renewal stops future renewals; it does not automatically refund amounts already paid. This policy does not limit any rights you have under applicable state or federal law.
6. Late payment. If a monthly or annual payment fails, we will notify you through your member portal and you will receive reminders while it remains unpaid. If the payment remains unpaid 7 days after it first fails, or by the day before the last day of the calendar month in which it failed, whichever is sooner, the membership will be terminated, automatic billing will stop, and eligibility for included services ends. No balance is accelerated and no penalty is charged. Access to your account and past invoices remains available, and you may contact Sprinkle Health about re-enrolling.
7. Availability. Memberships are offered throughout the United States except Maryland, Vermont, and Washington. Residents of Maryland, Vermont, or Washington cannot enroll in a Sprinkle Health membership.
8. Payment authorization. You authorize charges to your selected payment method as described at checkout.
